Volume 3 of Chemical Incidents Reports Now Available

The Chemical Safety and Hazard Investigation Board (CSB) has published a third volume of chemical incident reports. The new publication covers 30 incidents in chronological order starting with the April 2020 explosion at a paper mill in Maine and concluding with the accidental release of hot water in January 2025 at a paper mill in Mississippi. Together, the 30 incidents resulted in two deaths, 25 serious injuries, and approximately $1.8 million in property damage.

Like CSB’s previous volumes of incident reports, the new publication summarizes each event and identifies its probable cause. This information has not been previously made public, according to the agency. The reports are based on information CSB received in compliance with its accidental release reporting rule, which went into effect in March 2020. The rule requires reporting of any accidental release that results in a fatality, a serious injury, or substantial damage to property.

“This third volume of reports reflects the CSB’s continued commitment to keeping the American people informed about the serious chemical incidents that occur every week across this country and the risks that these incidents pose to communities and workers, as well as the enormous economic impact that they have on the American chemical industry,” said CSB Chairperson Steve Owens.
